I have read and understand the above terms of the agreement. I agree to the terms as set out in forms 1-3, and would like to enroll with the Cord Blood Bank of Canada. I understand that a copy of forms 1-5 will be forwarded to me with my personalized collection kit and I further
understand that my baby's cord blood, umbilical cord and placenta will not
be processed unless these forms have been completed and received to the CBBC. I agree to these terms and would like to proceed with enrollment.
