Read enrollment forms thoroughly, address any questions you may have with us, complete forms forms 1-5 and return them to the CBBC by fax or by mail. At your next scheduled physician/midwife visit please request that he/she faxes us your 1st set of prenatal blood test results as indicated in the “required blood work” file.
Enroll Online using our express online enrollment. Our express online enrollment is available 24 hours a day 7 days a week. Simply read through the forms, ask any questions and enroll. Once approved for enrollment your personalized collection kit will be released within 1-2 business days.
