In order to enroll in the Cord Blood Bank of Canada’s cord blood banking service, the following forms must be completed, signed and returned to our office ( mail or in person):
Form 1 Individual Account – Client Service Agreement.
The Client Service Agreement outlines the services provided by CBBC as well as the responsibilities of CBBC and the client
Form 2 Individual Account – Informed Consent for Collection and Storage of Cord Blood and Collection and Infectious Disease Marker Testing of Maternal Blood Sample.
The Informed Consent describes details regarding the collection, receipt, processing, storage, maintenance, and use of cord blood as well. It documents your understanding and consent to the services outlined as well as the collection of your baby’s cord blood by your caregiver.
Form 3 Individual Account – Limitation of Liability and Release
The Limitation of Liability and Release is provided for your caregiver and hospital.

Once you are enrolled we will release and ship your personalized collection kit to you well in advance of your expected due date. You will take your collection kit with you to the hospital at delivery.
